Once this form has been signed and returned, we will send you a user ID and password for each authorized staff member, the IESUB website address and the effective date of the security authorization. Once you are registered for this service, you can simply sign on to the IESUB website. Then, either key in your data or use the file transfer option to transmit your data in one of two specified file formats. |

| Federal Reserve Bank End User Authorization Contact (EUAC) Designation and Authorization Form for Reporting Central Services This form gives the EUAC authority to designate authorized individuals (Subscribers) to access the Reporting Central application using the FedLine Web access solution. When we receive the completed End User Authorization Contact Form, a credential will be issued to that EUAC. The EUAC will use the FedLine credential to access the EUAC Center, which enables the EUAC to reset FedLine Subscriber passwords and access Subscriber reports and FedLine documentation once credentials have been issued to your organization's Subscribers. |

| Federal Reserve Bank External Subscriber Credential and Access Control Request Form for Reporting Central Services An organization's EUAC should complete a separate Federal Reserve Bank External Subscriber Request Form for each individual who will be authorized to access Reporting Central Services. This form authorizes the Federal Reserve Banks to create a credential granting the Subscriber access to the Reporting Central Services indicated on the form. NOTE: The EUAC will receive an acknowledgement email from the Federal Reserve confirming the receipt of the request. |
